MailScanner (and other email security systems) can impose limits on the number of email messages coming from any individual IP address in a block of IP addresses. Using this, you can easily throttle connections from Zombie PC's, as no clean PC is likely to generate more than 30 or so email messages per hour. A zombie will generate more than that, and so its output rate will be throttled down to 30 messages per hour.
Of course all of this can be configured in MailScanner (and others).
This really isn't a big problem, it is very easy for ISPs to solve.
