Story: Government department loses £1.5bn through IT
Looks to me that they went ahead without actually knowing what they had, where it was, how it worked, what is important, how to fall-back to the last known good situation or even how to determine what is and isn't good for them before implementing it.
Yet, despite indications of the opposite, they seem content enough to carry on the choosen path regardless.
As they say. Some people sell dreams. Some people live dreams. Some people hope they're dreaming and the rest? Well, they simply have to coupe, don't they?
